Here is the information you will need to provide so we are authorized to pick up your child(ren) for our afterschool programs.
For New York Society of Play
We must collect the following information for your child(ren)’s classes:
- Days of Enrollment in Afterschool
- Class Number
Please fill out our Google Form to provide us with this information.
NOTE: In the spot for “Child Teacher Name,” please include your child teacher’s name AND class number. This information is required by your school.
If you have already filled out this form but did NOT provide your student’s class number, you can email us with this information at [email protected] without filling out the form a second time.
For PS 110
PS 110 must receive a form of consent from parents if they are attending an after school program and being picked up at the school by that organization. You should have received this form by email from your school’s parent coordinator. Please make sure to fill out this form and have your child bring it to their teacher before their first day of afterschool.
If you do not have a copy of this form, please reach out to the PS 110 administration in order to obtain one.
